Health Choice Generations (HMO D-SNP) members have access to EXTRA BENEFITS to help inspire better health for our members at NO COST!

On January 1, 2021, our plan name will change from Health Choice Generations to Health Choice Pathway.

You receive a $3000 allowance every year towards preventive and comprehensive dental services like dentures, crowns, fillings, extractions and endodontics. Other covered services include: 2 oral exams every year; 2 x-rays every year; 2 cleanings every year; 1 fluoride treatment every year. For help finding a dental provider, contact Member Services.

Silver&Fit Fitness Benefit

Health Choice Generations knows how important fitness is to our members. During this unprecedented health crisis, we want to support you with additional resources to keep you on track. With the Silver&Fit fitness benefit, we’ve got Something for Everyone® and is available right on the Silver&Fit website at www.SilverandFit.com.

The Silver&Fit Exercise & Healthy Aging Program provides you with the following services:

  • You will receive a fitness center membership.
  • Available fitness center types include full centers, basic coed centers, YMCAs, gender specific centers, and exercise centers.
  • Fitness advisors at Silver&Fit contracted centers will meet with you and introduce you to the fitness center and assist you with enrolling.

Fitness Center Program. Your fitness membership gives you access to more than 14,000 fitness locations across the country with all basic gym features. Many fitness centers offer group classes for older adults.

Home Fitness Kits. If you like to work out at home, choose up to 2 kits per benefit year with up to 35 unique options available, including a Fitbit® Connected! Kit.

2020

You receive a $1500 allowance every 3 years for hearing aids, per ear, 1 evaluation/fitting every 3 years and 1 routine hearing exam every year. For help finding a hearing provider, contact Member Services.

2021

You receive a $1500 allowance every year for hearing aids, for both ears combined, 1 evaluation/fitting every year and 1 routine hearing exam every year. For help finding a hearing provider, contact Member Services.

 


2020 You receive a $150 allowance every 3 months towards over-the-counter health items such as toothpaste, bandages, antacid, sunscreen, cold medicine, pain relievers and much more!

2021

You receive up to 24 additional one-way transportation trips every calendar year. If you need transportation to and from plan-approved locations, including medical appointments, call 1-888-418-0903 (TTY 711), 24 hours a day, 7 days a week. Please call 72 hours before your appointment to schedule transportation. If you have an urgent medical need, please call to arrange transportation. For emergency medical needs, please call 911.

You can talk to a primary care provider, specialist or urgent care from the comfort of your own home 24 hours a day, 7 days a week. Not all medical conditions can be treated through virtual visits.

You receive a $300 allowance towards your choice of contacts and/or eyeglasses (lenses and frames) every year and 1 routine eye exam every year. For help finding a vision provider, contact Member Services.

2021

You receive a $350 allowance towards your choice of contacts and/or eyeglasses (lenses and frames) every year and 1 routine eye exam every year. For help finding a vision provider, contact Member Services.
