You may receive a survey in the mail asking questions about the healthcare BCBSAZ Health Choice Pathway (HMO D-SNP) gives you. Please take the time to complete this survey.

Our goal is to give you quality healthcare, programs, services, and benefits to support you on your path to wellness.

You will be asked questions like:

  • How easy is it to get an appointment with your doctor or fill a prescription at your pharmacy?
  • As your health plan, does our Member Services team help answer your questions?
  • How do you rate us as your health plan on a scale of 0-10?

And what will we do with your feedback?

As your health plan, we will keep working to make sure we meet your needs by:

  • Working with providers to help you get appointments when you need them
  • Answering your health plan questions with a Member Services team and personal Health Care Buddy who is there for you
  • Supporting you with physical and mental health and wellness programs
